ความเหลื่อมล้้าของระบบสุขภาพ 3 กองทุน

บทคัดย่อ
การวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อศึกษาสภาพการณ์และปัญหาความเหลื่อมล้ำของระบบสุขภาพ 3 กองทุน โดยใช้วิธีการวิจัยเชิงคุณภาพเก็บข้อมูลจากการวิเคราะห์เอกสารและสัมภาษณ์เชิงลึกจากผู้ให้ข้อมูลหลัก 25 คน ผลจากการวิจัยพบว่าความเหลื่อมล้ำของระบบสุขภาพมี 4 ด้าน ได้แก่ 1) ด้านการเงินการคลังโดยรัฐให้เงินอุดหนุนแต่ละกองทุนไม่เท่ากัน 2) ด้านการบริหารจัดการกองทุนโดยแต่ละกองทุนมีการบริหารจัดการที่แตกต่างกัน เช่น ระบบการจ่ายเงินค่ารักษาพยาบาลของสถานพยาบาลเป็นต้น 3) ด้านสิทธิประโยชน์แต่ละกองทุนมีเงื่อนไขการรักษาที่แตกต่างกันสำหรับการรักษาโรคเดียวกันในบางรายการ รวมทั้งมีทางเลือกในการรักษาที่แตกต่างกันบางรายการจากการใช้ยา เวชภัณฑ์ อุปกรณ์ และอวัยวะเทียม และ 4) คุณภาพการรักษาและการบริการพบว่าสวัสดิการรักษาพยาบาลจะได้รับความพึงพอใจมากที่สุด รองลงมา คือ กองทุนประกันสังคม และหลักประกันสุขภาพแห่งชาติ ดังนั้นรัฐบาลควรมีกลไกกลางควบคุมทั้ง 3 กองทุนให้มีการบริหารจัดการทิศทางเดียวกัน และประชาชนทุกสิทธิควรได้รับบริการทางด้านสุขภาพอย่างเท่าเทียมและเป็นธรรมโดยมีชุดสิทธิประโยชน์จำเป็นเหมือนกัน
