This study aims to study the level of factors affecting professional competency in Thai traditional medicine, develop a multilevel structural equation model of factors affecting professional competency in traditional medicine, and investigate the factors affecting professional competency in Thai traditional medicine. The sample consisted of 90 teachers and 1,050 fourth-year students, selected by multi-stage random sampling. The data were collected by using a questionnaire and analysed by using mean, standard deviation, and multilevel structural equation models. The research results revealed that overall level of factors affecting professional competency in Thai traditional medicine at the individual and university levels was high.  The results of the multilevel structural equation model analysis were consistent with the empirical data, with Chi-square= 448.988, df = 251, x2  /df=1.79 RMSEA =0.027  CFI =0.963, TLI = 0.955 SRMRw = 0.042 , and SRMRb =0.259. It was found that the factors at the university level affecting professional competency in Thai traditional medicine were: teacher spirit with a value of 1.000, perception of teachers' abilities with a value of 2.401, educational institution management with a value of 1.000, curriculum management with a value of 0.429, and academic environment Physical with a value of 0.454. In addition, in terms of the factors at the individual level, it was found that the attitudes towards learning had the size of influence with a value of 1.000, achievement motivation with a value of 1.471, confidence in one's own power with a value of 2.059, social support with a value of 1.000, teaching and learning with a value of 0.973, service workers with a value of 1.000, service operations with a value of 0.689, and service environment with a value of 0.877.
