การบริหารจัดการระบบสาธารณสุขทางทะเลของโรงพยาบาลบนเกาะ ในพื้นที่ห่างไกล

บทคัดย่อ
บทความนี้มีวัตถุประสงค์เพื่อ 1) ศึกษารูปแบบการบริหารจัดการระบบสาธารณสุขทางทะเลของโรงพยาบาลบนเกาะในพื้นที่ห่างไกล 2) ศึกษาปัญหาและอุปสรรคของการบริหารจัดการดังกล่าว การวิจัยครั้งนี้เป็นการวิจัยเชิงคุณภาพ มีผู้ให้ข้อมูลสำคัญจำนวน 30 ราย ซึ่งเป็นบุคคลที่เกี่ยวข้องกับการบริหารจัดการระบบสาธารณสุขทางทะเล เครื่องมือที่ใช้เก็บรวบรวมข้อมูล ได้แก่ แบบสัมภาษณ์ ผลการวิจัยพบว่า 1) โรงพยาบาลบนเกาะพื้นที่ห่างไกลจำนวน 6 แห่งในภาคใต้ ได้แก่ โรงพยาบาลเกาะสมุย เกาะพะงัน เกาะเต่า เกาะยาวชัยพัฒน์ เกาะพีพี และเกาะลันตา มีรูปแบบการบริหารจัดการที่คล้ายคลึงกัน แต่ศักยภาพจะต่างกันตามประเภทและสถานะของโรงพยาบาล การบริหารจัดการกรณีเหตุฉุกเฉินจะเริ่มต้นเมื่อได้รับแจ้งเหตุ ฝ่ายที่เกี่ยวข้องจะประสานไปยังตำรวจน้ำและทีมแพทย์ฉุกเฉิน เพื่อส่งเจ้าหน้าที่ไปยังจุดเกิดเหตุและให้การช่วยเหลือเพื่อนำส่งโรงพยาบาลต่อไป การลำเลียงผู้ป่วยจะพิจารณาเลือกใช้ยานพาหนะตามความเหมาะสม ได้แก่ เรือจากภาคเอกชน เรือเร็ว เรือจากกองทัพ เรือของศูนย์อำนวยการรักษาผลประโยชน์ของชาติทางทะเล รวมถึงเฮลิคอปเตอร์ การบริหารจัดการดังกล่าวจะเป็นลักษณะของการทำงานที่หน่วยงานต่าง ๆ บูรณาการร่วมกัน 2) การให้บริการทางสุขภาพของโรงพยาบาลพบว่า ปัญหาและอุปสรรคในการบริหารจัดการ ได้แก่ ข้อจำกัดด้านการขนส่งและการเดินทาง การขาดแคลนบุคลากรทางการแพทย์ โดยเฉพาะแพทย์เฉพาะทางด้านเวชศาสตร์ทางทะเลและเวชศาสตร์ฉุกเฉิน รวมถึงการกำหนดอัตรากำลังบุคลากรที่ไม่สอดคล้องภาระงานของกระทรวงสาธารณสุข การขาดแคลนทรัพยากรทั้งในด้านอุปกรณ์ทางการแพทย์และระบบเทคโนโลยี รวมถึงการบุคลากรบางรายยังขาดทักษะที่จำเป็น
